A self-evaluation is usually counter checked with the reports given by the immediate supervisor of the person who have done it. It will be a good thing if both evaluations show accurate and cohesive results. However, if there are discrepancies, human resource may set an appointment to clear out matters and why certain differences have occurred.
A self-evaluation usually includes:
- Specific works that an employee has done that sets him or her from other employees
- The knowledge of the employee in terms of his or her daily work functions
- Work-related accomplishments that an employee was able to achieve
- The achievements of the department where an employee belongs and how has he or she been a key factor in achieving it
- The personal goals of the employee in terms of professional growth
- The means and ways on how an employee will reach those goals
- How the employee perceiving the processes of the company is
- The current standing of the employee in terms of his or her functions
- The things that the employee want to improve both personally and for the organization

Remember that there are counterchecks to be done and putting items that are not real just to up your professional game may be the reason why things will turn the other way around.
Being honest in answering also will let the human resource department know that you are aware of your strengths and weaknesses. This means that you are trying to find ways on how to show our strengths and for them to be of good use to the company. And also, as you already know your weaknesses, you are trying to be better by improving on those areas.
A performance evaluation made by oneself will show his or her character more than what he or she thinks of the job that he or she is doing. If you think that you deserve good reviews, then right them down. Be proud of your professional work ethics and achievements. If not, then just practice honesty and make sure that you will do better so that you have good things to write on your next self-evaluation.
