Good day!

This letter is in regards to the medical insurance (account number 34232) that I hold with your insurance company. I would like to request a claim for the medical expenses that I incurred owing to my medical treatment.

Recently I had a kidney problem which required immediate operation. I was admitted to St Thomas Hospital which is located at 34 Ropealley Street. My surgeon was Dr Peter Watson. As per the insurance policy I can claim 90% of the medical expenses. The total bill came out to be $3700.

I would request you to consider my appeal at the earliest. Please find enclosed the relevant documents for your reference. Kindly let me know if you require any other documents.
