I hope this letter finds you well. I am writing to provide my consent for my child, Jane Smith, who is in the fourth grade, to participate in the upcoming field trip organized by Cottril Public School. I understand that the field trip is scheduled for May 15, 2050, and the destination is the Quincy Science Museum.
I have reviewed the details of the field trip and am aware that the activities may include guided tours, interactive exhibits, and a supervised lunch. I am confident that the school has taken appropriate measures to ensure the safety and well-being of the students during the trip.
In case of any medical concerns, please be aware that my child has a mild peanut allergy. Additionally, I have attached a copy of my child's emergency contact and medical information for your records.

I hereby release Cottril Public School, its staff, and any volunteers from any liability in the event of an accident or injury during the field trip.
