Dear Ms. Williams,

I am writing to formally request a leave of absence from my duties starting from January 18, 2055, to January 23, 2055. A close family member of mine is gravely ill, and I have made the decision to spend these crucial moments by their bedside. In an effort to minimize any inconvenience that may come with my absence, I have allotted time to delegate my duties to my team mates.

I understand that this period is a busy time for the company, and I would not have planned this leave if it were not a matter of life and death. I greatly appreciate your kind understanding and consideration in this matter. I will make sure to provide any updates if necessary and will be checking my email occasionally for any updates or urgent matters.
