I hope this letter finds you in good health. I am penning this letter to formally request for a vacation leave from work for health reasons. Specifically, the planned duration of my leave will be from July 23, 2085, to July 26, 2085.

In recent times I have been experiencing some difficulties in maintaining my mental health balance due to work pressure and personal life issues. A therapist I am consulting has suggested taking a short vacation as a part of the solution. Hence, I have decided to take this short vacation to focus on my mental well-being.
