Job Title: Adaptation Nurse
Our health care institution is searching for a highly compassionate and dedicated Adaptation Nurse. The core obligation of this role entails ensuring the successful transition of patients from hospital care to home care or community care, with a particular focus on elderly and chronically ill patients. The Adaptation Nurse will work closely with patients, families, and caregivers to develop suitable care plans.
Responsibilities and Duties:
As an Adaptation Nurse, you will assess patients' needs and develop adaptive care plans, coordinate with medical professionals about these plans, educate the patient and their caregivers, and monitor the patient's progress and adapt the care plan as needed. In addition, you will be responsible to ensure ancillary services such as physical therapy and social services are appropriately scheduled and executed.
Qualifications, Skills, and Competencies:
The job demands you to hold a valid Registered Nurse license and have a Bachelor of Science in Nursing. Previous experience dealing with elderly or chronically ill patients is preferred. Strong interpersonal, communication skills and the ability to work well in a team are integral to this role. Being compassionate, empathetic and patient, with good problem-solving capabilities are keys to succeed in this position.
Experience:
A minimum of two years experience working as a certified Registered Nurse, preferably in the field of geriatrics or chronic illness nursing. Familiarity with home healthcare or community healthcare services will be an added advantage.
