Introduction
This hospital note serves as an official record of a recent medical visit by a patient. It details the findings, observations, and subsequent recommendations made due to the visit. This document is instrumental in informing accommodative modifications required at a workplace to promote the patient's healing process, wellness, and overall wellbeing.
Observations
-
Patient arrived with complaints of persistent low-grade fever, mild headache, and fatigue.
-
Initial vital signs were within normal range, excluding slightly elevated body temperature.
-
No visible signs of distress or pain were observed.
-
Patient was cooperative and lucid during the examination.
Assessment
The patient's symptoms suggested possible viral infection which required further testing for proper diagnosis. A full blood count, urinalysis, and viral screening were subsequently recommended by the physicians.
Findings
-
Blood Count: WBC within normal range, slight increase in lymphocytes.
-
Urinalysis: All well within normal limits.
-
Viral Screening: Indicated presence of Epstein-Barr virus (EBV), consistent with acute infectious mononucleosis.
Recommendations
Upon diagnosis of acute infectious mononucleosis, a treatment plan and work modifications have been suggested. These include but are not limited to rest, adequate hydration, and avoidance of strenuous physical activities.
Given the nature of this condition, the patient may require time away from work to recover fully. I recommend that the patient avoid strenuous physical activities for a period of 3-4 weeks or until further assessment.
Enclosure:
Attached are laboratory results to verify the medical examination and subsequent diagnosis.
