Industry Event Participation Consent Form

Legal Protection:

I, Grace Dawson, an employee of Human Resources, acknowledge that my participation in the industry-related event titled "Tech Innovations Conference 2050" is entirely voluntary and of my own accord. I understand there may be risks and liabilities associated with attending this event.

•  I am aware of and accept these potential risks.

•  I understand the Company shall not be held responsible or liable for any loss, injury, or damage that may occur as a result of my participation.

Information Sharing:

By participating in the "Tech Innovations Conference 2050", I agree to share relevant information and knowledge acquired during the event with my employer, Human Resources, and my colleagues as deemed appropriate and beneficial to the Company. I will not disclose any proprietary or confidential information belonging to the Company.

•  I agree to share knowledge gained at the event.

•  I will uphold confidentiality agreements and policies.

Company Policies:

I commit to upholding the Company's policies and expectations during my participation in the "Tech Innovations Conference 2050". I understand that representing the Company at this event requires me to:

•  Conduct myself professionally and respectfully.

•  Adhere to the Company's code of conduct, ethics, and values.

•  Follow the Company's expense reimbursement procedures for any expenses incurred during the event.

I understand that violations of the Company's policies may result in disciplinary action.

Liability Waivers:

I acknowledge that the nature of the "Tech Innovations Conference 2050" may involve certain risks, including but not limited to travel-related risks, exposure to new ideas and concepts, and interactions with individuals external to the Company. In consideration of being allowed to participate in this event, I hereby release and discharge Human Resources and its officers, employees, and representatives from any liability, claims, demands, actions, or causes of action that may arise out of or in connection with my participation in the event.

•  I have read and understand the liability waiver statement.

Use of Likeness:

I understand that the "Tech Innovations Conference 2050" may involve photography or videography. By participating in the event, I grant Human Resources permission to use my likeness, including photographs and videos, in promotional materials or on social media related to the Company or the event.

•  I consent to the use of my likeness in promotional materials or on social media.

Expense Reimbursement:

If applicable, I understand that the Company has policies in place for the reimbursement of expenses incurred during the "Tech Innovations Conference 2050." I will adhere to the following guidelines for expense reimbursement:

•  I will submit expense reports and receipts in a timely manner, in accordance with the Company's reimbursement policy.

•  I will only claim expenses that are directly related to and necessary for my participation in the event, such as travel, accommodation, or registration fees.

•  I understand that any expense reimbursement is subject to approval by the Company and compliance with the Company's expense policies.
