Professional Service Agreement
This Professional Service Agreement ("Agreement") is made and entered into this first day of January 2050, by and between Dr. Vyn King (referred to hereinafter as the "Psychologist") and Jeremy Pitman (referred to hereinafter as the "Client").
1. Services
The Psychologist agrees to provide psychiatric evaluation and therapy/rehabilitation services to the Client on an individualized basis. Services will include, but are not limited to, diagnostic assessments, treatment planning, psychotherapy, and further referrals if needed.
2. Term
The Agreement shall commence on January 1st, 2050, and continue until terminated by either party with written notice of 30 days.
3. Payment
The Client agrees to pay the Psychologist a fee of $200 per session. Payment is due at the time of service unless otherwise agreed upon. If needed, adjustments can be made to this fee schedule at the discretion of the Psychologist.
4. Confidentiality
The Psychologist will maintain the confidentiality of all information received from the Client in accordance with applicable laws and ethical standards. Exceptions to this confidentiality include potential harm to self or others, child abuse, elder abuse, or where otherwise required by law.
5. Termination
Either party may terminate this Agreement at any time upon giving 30 days prior written notice. The Psychologist will make every effort to assist the Client in finding alternate care if necessary.
6. Governing Law
This Agreement will be governed by the laws of the State in which the Psychologist practices.
7. Entire Agreement
This Agreement constitutes the entire agreement between the parties and supersedes any prior oral or written agreements. If any part of this Agreement is deemed unenforceable, all other parts shall remain in effect.
By signing below, both the Psychologist and the Client acknowledge and agree to all terms and conditions of this Agreement.
