DAYCARE CONTRACT
This Daycare Contract is made and entered into by and between [YOUR NAME], herein referred to as the “Provider,” with a principal place of business located at [YOUR COMPANY ADDRESS], and [Parent/Guardian’s Name(s)], herein referred to as the “Parent/Guardian,” residing at [Parent/Guardian’s Address].
I. Services Provided
The Provider agrees to offer daycare services for the child(ren) named below, under the terms and conditions outlined in this Contract.

II. Duration of Services
This Contract is effective as of April 1, 2050, and will continue until terminated by either party with a notice period, of two-week written notice. The daycare services will be provided from 8:00 AM to 5:00 PM, Monday through Friday, excluding public holidays.
III. Payment Terms
The Parent/Guardian agrees to a monthly fee of $800 payable on the first day of service each month. This fee covers all regular daycare services, including Meals, educational materials, and outdoor play equipment.
Late Payment Policy: Payments received after the due date will incur a late fee of $25.
IV. Sick Child Policy
Parents/Guardians agree not to bring their child(ren) to the daycare if exhibiting signs of illness such as fever, vomiting, or contagious conditions. The Provider reserves the right to refuse service to a sick child for the health and safety of all children in care.
V. Medication Administration
If medication administration is necessary during daycare hours, the Parent/Guardian must provide written consent and instructions. Medications must be in their original packaging with the child’s name clearly labeled.
VI. Emergency Procedures
In case of an emergency, the Provider will attempt to contact the Parent/Guardian immediately. If the Parent/Guardian cannot be reached, the Provider is authorized to take necessary action for the child’s well-being, including medical treatment if needed.
VII. Termination of Services
Either party may terminate this Contract with a [notice period, e.g., two-week] written notice. The Provider may terminate the Contract immediately under certain conditions, including but not limited to non-payment, repeated late pick-ups, or failure to comply with the Contract terms.
VIII. Confidentiality
Both parties agree to keep information related to this Contract and any personal information of the children confidential, except as required by law or for the child’s safety and welfare.
IX. Liability
The Parent/Guardian acknowledges that while the Provider takes all reasonable steps to ensure the safety and well-being of the child(ren), the Provider cannot be held liable for any accidents or injuries that may occur during the term of this Contract, except in cases of gross negligence or willful misconduct.
X. Amendments
This Contract can only be amended in writing with the consent of both parties.
