This Managed Service Provision Agreement (henceforth "Agreement") is entered into as of this day of [Day/ Month/ Year] (henceforth "Effective Date"), between [Your Company Name], having its principal place of business at [Your Company Address], referred to as the "Client" herein, and [Service Provider Company Name], having its principal place of business at [ServiceProvider Company Address], referred to as the "Service Provider" herein.
1. Scope of Services

2. Service Level & Standards

3. Pricing

5. Confidentiality
