Introduction
The purpose of this form is to provide a means for evaluating the level of preparation that has taken place before a lung transplant procedure and also to assess the outcome or result of the procedure once it has been executed.

This evaluation provides a well-organized assessment where we thoroughly examine and evaluate the health of the patient before and after the transplant procedure. Furthermore, it also enables us to determine the success rate of the transplant procedure.
Evaluation Criteria
Criteria for this evaluation include the patient's health status pre- and post-transplant, surgical procedure effectiveness, post-operation recovery, and lung function. Each criterion is articulated & assessed in detail in the evaluation table.
