1. Parties
This Caregiver Contract (the "Contract") is made and entered into on [Effective Date] (the "Effective Date"), by and between [YOUR NAME], an individual, of [Your Company Address] (the "Caregiver"), and [CLIENT'S NAME], an individual, of [Client's Address] (the "Client").
Hereinafter, Caregiver and Client may be collectively referred to as the "Parties."
2. Duties of Caregiver
Caregiver agrees to provide caregiving services to the Client as described in this Contract. Such services may include, but are not limited to, housekeeping, meal preparation, personal care, and companionship. The specific tasks and responsibilities of the Caregiver are outlined in Schedule A attached hereto.
3. Hours of Work
The Caregiver will provide services to the Client [Number of hours per week] per week, and their daily schedule and duties will be arranged between the Parties as needed. Any hours worked beyond the agreed-upon hours may be subject to overtime pay at a rate of [Overtime Rate] per hour.
4. Compensation

The Caregiver agrees to indemnify and hold harmless the Client from any claims, losses, and damages related to the performance of this Contract, except for those arising from the Client's negligence or intentional misconduct.
9. Governing Law

10. Entire Agreement
This Contract constitutes the entire agreement between the parties with respect to the subject matter hereof and supersedes all previous written or oral agreements between the parties.
