Personal Trainer Contract

This Personal Trainer Contract (the "Agreement") is made and entered into as of [Date], by and between:

[Client Name] (the "Client"), residing at [Client Address]

and

[Trainer Name] (the "Trainer"), residing at [Trainer Address]

I. Services Provided

The Trainer agrees to provide personal training services to the Client in accordance with the terms and conditions outlined in this Agreement. The services may include but are not limited to fitness assessments, personalized workout plans, nutritional guidance, and motivation.

II. Term

This Agreement shall commence on [Start Date] and shall continue until terminated by either party in accordance with the provisions of this Agreement.

III. Payment

In consideration for the services provided, the Client agrees to pay the Trainer [Payment Amount] per session. Payment shall be made [weekly/monthly/upon completion of each session] and is due in advance of each session.

IV. Cancellation Policy

The Client agrees to provide at least 24 hours notice in the event of cancellation or rescheduling of a training session. Failure to provide adequate notice may result in forfeiture of the session fee.

V. Confidentiality

The Trainer agrees to keep all information disclosed by the Client confidential, including but not limited to health history, fitness goals, and personal information. The Trainer will not disclose this information to any third party without the Client's express consent unless required by law.

VI. Assumption of Risk

The Client acknowledges that participation in personal training sessions involves certain inherent risks, including but not limited to muscle soreness, strains, and sprains. The Client agrees to assume all risks associated with participation in the training sessions and hereby releases the Trainer from any and all liability arising out of such participation.

VII. Indemnification

The Client agrees to indemnify and hold harmless the Trainer from any and all claims, damages, losses, or liabilities arising out of or related to the Client's participation in the training sessions.

VIII. Termination

Either party may terminate this Agreement at any time upon written notice to the other party. In the event of termination, the Client shall be entitled to a refund of any prepaid session fees for sessions not yet provided.

IX. Governing Law

This Agreement shall be governed by and construed in accordance with the laws of [Jurisdiction]. Any dispute arising out of or relating to this Agreement shall be resolved exclusively in the courts of [Jurisdiction].

X. Entire Agreement

This Agreement constitutes the entire understanding and agreement between the parties with respect to the subject matter hereof and supersedes all prior agreements, whether written or oral, relating to such subject matter.
