JCT CONTRACT

This contract ("Contract") from here forward is entered into as of [Date], by and between [Contractor Name], henceforth referred to as "Contractor," with an address at 2471 Lodgeville Road Minneapolis, MN 55402, and [Client Name], henceforth referred to as "Client," with an address at 65 Lowes Alley Columbus, OH 43201.

1. Scope of Work
