This Contract is entered into this day of [DATE], between [Your Name], hereinafter referred to as "Makeup Artist", and [Client Name], hereinafter referred to as "Client", both of whom agree to be bound by this Agreement.
The individual who is professionally skilled and employed as a Makeup Artist currently lives at the address known as [Your Company Address], while the person who has engaged the services of the Makeup Artist, referred to as the Client, has their place of residence at the location which is [Client Address].
The individual who is identified as the Makeup Artist has the obligation to deliver their professional services in makeup artistry to the Client. These services will adhere to the specific requirements as set out by the Client. Additionally, the provision of these services will occur on a date that is mutually agreed upon by both the Makeup Artist and the Client.
The Client agrees to pay the Makeup Artist the sum of [AMOUNT] for the Services provided. The agreed payment will be made on [Payment Date].
The individual providing the makeup artistry services functions in the capacity of an independent contractor, and therefore, does not hold the status of being an employee under the management of the client.
The Makeup Artist will not be held responsible for any reactions due to allergies or sensitivities to the products used. It is the Client's responsibility to notify the Makeup Artist of any allergies or sensitivities before the services being rendered.
If the Client needs to cancel the makeup services for any reason, a written notice must be provided to the Makeup Artist at least [number of days] days before the scheduled event.
