SPAY/NEUTER CONTRACT

This Spay/Neuter Contract ("Contract") is entered into on [Date] by and between [Pet Owner's Name], hereinafter referred to as the "Owner," and [Veterinary Clinic's Name], hereinafter referred to as the "Veterinary Clinic."

1. AGREEMENT

THE PARTIES AGREE AS FOLLOWS:

1. The Owner warrants that he/she is the legal owner of the pet who is the subject of this Contract.

2. The pet to be spayed/neutered under this Contract will be described as follows: Name: [Pet's Name]; Breed: [Pet's Breed]; Age: [Pet's Age]; Color: [Pet's Color].

3. The Owner has voluntarily chosen to have the pet spayed/neutered and represents that to the best of his/her knowledge and belief, the pet is in good health and is not a litter-bearer nor pregnant.

4. The Veterinary Clinic agrees to perform the spay/neuter surgery for the pet as per the terms outlined in this Contract.

5. The Owner understands and accepts that all surgical procedures involve some risk to the pet and agrees to the use of anesthesia and other medications necessary for the procedure.

6. The Owner agrees to provide complete and accurate medical information about the pet prior to the surgery.

7. The Veterinary Clinic agrees to inform the Owner of the inherent risks involved in the procedure and to take all reasonable precautions to minimize such risks.

8. The Veterinary Clinic agrees to provide post-operative instructions to the Owner which must be followed for the successful outcome of the surgery.

9. The Owner, by signing this Contract, affirms that he/she has read and fully understands the terms outlined herein.

10. The Veterinary Clinic represents that it is licensed to practice veterinary medicine and that all services will be performed in a competent, professional manner in accordance with related practice standards.

2. DISCLAIMER OF WARRANTY

The Veterinary Clinic makes no warranty, expressed or implied, regarding the success or failure of the surgery performed under this Agreement.

3. OWNERSHIP AND CONTROL

The Owner will maintain full control, care, and custody of the pet at all times, except for the time during which the pet is under the care of the Veterinary Clinic for the purposes of the procedure contemplated by this Contract.

4. PAYMENT

The Owner agrees to pay [Amount] to the Veterinary Clinic for the procedure. Payment must be made in full no later than [Payment Date].

5. DISPUTE RESOLUTION

In the event that a dispute arises from this Contract, it shall be resolved through mediation. If a resolution cannot be reached, the parties agree to enter into binding arbitration.

6. ENTIRE AGREEMENT

This Contract sets forth the entire agreement between the Owner and the Veterinary Clinic and supersedes all prior oral or written agreements, understandings, or arrangements relating to this subject matter.
