HOME HEALTH CARE CONTRACT
This Home Health Care Contract (the "Contract") is made effective as of [Date], by and between [Your Company name], with a registered address at [Your Company Address] ("Service Provider"), and [Client's Name], residing at [Client's Address] ("Client").
1. SCOPE OF SERVICES
The Service Provider agrees to furnish home health care services ("Services") to the Client as outlined herein. The Services include but are not limited to, the following: personal care assistance (bathing, grooming, and dressing), medication management, light housekeeping, meal preparation, mobility assistance, transportation to medical appointments, and companionship. The Service Provider commits to delivering these Services with the utmost professionalism, skill, and care under industry standards and applicable laws.
2. TERM AND TERMINATION
This Contract is effective from the Effective Date and will remain in effect until December 31, 2050, unless terminated earlier under this section. Either party may terminate this Contract by providing 30 days written notice to the other party.
Upon termination, the Service Provider shall be compensated for all Services rendered up to the termination date that have not yet been paid.
3. COMPENSATION
For the Services provided under this Contract, the Client agrees to pay the Service Provider a total fee of $3,500 per month. Payments shall be made on the 5th day of each month. Any late payments shall incur a late fee of $50 unless otherwise agreed upon in writing.
4. SPECIFIC CARE REQUIREMENTS
The Service Provider shall comply with all specific care requirements as provided by the Client or their designated medical professional. These requirements include, but are not limited to: regular monitoring of blood pressure and blood sugar levels, administration of insulin or other prescribed medications, dietary restrictions including low sodium and sugar-free meals, physical therapy exercises three times a week, and the use of a medical alert system for emergencies. Any changes or updates to these requirements must be communicated in writing and agreed upon by both parties.
5. CONFIDENTIALITY
Both parties pledge to maintain the confidentiality of all information related to the Client, the Services, and the terms of this Contract. This confidentiality clause shall survive the termination or expiration of this Contract.
6. INDEMNIFICATION
Each party agrees to indemnify and hold harmless the other from any claims, losses, or expenses, including attorney's fees, arising from their obligations under this Contract, excluding gross negligence or willful misconduct. This obligation includes any damages from the services provided or any breach of this Contract.
7. ENTIRE AGREEMENT
This Contract represents the entire agreement between the Service Provider and the Client regarding its subject matter, superseding all previous agreements, understandings, promises, and representations, whether written or oral. Any amendments to this Contract must be made in writing and signed by both parties.
