1. Introduction

On May 14, 2054, a Dental Hygienist Contract, henceforth known as the "Contract," was negotiated and agreed upon between [Dental Practice Name] at [Address], subsequently referred to as the "Practice," and [Dental Hygienist Name], who lives at [Address], hereinafter referred to as the "Hygienist."

2. Scope of Services

2.1 Services to be Provided The Hygienist agrees to provide dental hygiene services to patients of the Practice, including but not limited to teeth cleaning, oral health education, and preventive dental care.

2.2 Schedule The Hygienist shall perform services on the days and hours mutually agreed upon by both parties. Any changes to the schedule shall be communicated and agreed upon in writing.

2.3 Location The services shall be performed at the Practice's dental office located at [Address] unless otherwise agreed upon by both parties.

3. Terms of Employment

3.1 Employment Status The Hygienist shall be considered an independent contractor and not an employee of the Practice. The Hygienist shall not be entitled to employee benefits, including but not limited to health insurance, retirement benefits, or paid time off.

3.2 Compensation The Practice shall compensate the Hygienist at a rate of [Rate] per hour/day, payable [Frequency of Payment], for services rendered. Payment shall be made via [Payment Method] on [Payment Schedule].

4. Confidentiality

4.2 Non-Disclosure Agreement The Hygienist shall sign a separate non-disclosure agreement (NDA) provided by the Practice, outlining the specific terms and conditions regarding the protection of confidential information.

4.3 Return of Property Upon termination of this Contract, the Hygienist shall return any property belonging to the Practice, including but not limited to patient records, equipment, and supplies.

5. Governing Law

This Contract shall be governed by and construed under the laws of [Jurisdiction]. Any disputes arising out of or in connection with this Contract shall be resolved through arbitration in [Jurisdiction].

6. Entire Agreement
