I. Introduction

This Release of Liability for Minor Participants ("Release") is entered into between [Your Name] ("Organizer") and the undersigned by [Parent/Guardian Name] ("Parent/Guardian") on behalf of the minor participant named herein ("Minor Participant"). VI. Severability
