Youth Sports Waiver and Release of Liability
I. Parties Involved
This Agreement was made on [Date] between [Your Company Name], located at [Your Company Address], hereinafter referred to as "the Organization," and [Participant's Name], residing at [Participant's Address], or parent/legal guardian thereof, hereinafter referred to as "Participant" or "Parent/Guardian."
In consideration of the Organization allowing the Participant to participate in youth sports activities, the undersigned agrees as follows:
II. Assumption of Risks
The Participant and/or Parent/Guardian understands that participation in youth sports activities involves inherent risks, including but not limited to physical injury, illness, disability, or death. The Participant and/or Parent/Guardian acknowledges and assumes all such risks, whether known or unknown.
III. Release of Liability
The Participant and/or Parent/Guardian hereby releases, waives, discharges, and covenants not to sue the Organization, its directors, officers, employees, volunteers, coaches, and agents (collectively referred to as "Releasees") from all liability, claims, demands, actions, or rights of action arising out of or related to any loss, damage, or injury, including but not limited to bodily injury, disability, or death, which may be sustained by the Participant during or arising out of participation in youth sports activities.
IV. Indemnification
The Participant and/or Parent/Guardian agrees to indemnify and hold harmless the Releasees against any claims, demands, losses, or expenses, including attorney's fees, arising from or related to the Participant's participation in youth sports activities.
V. Medical Treatment Authorization
The Participant and/or Parent/Guardian authorizes the Organization and its agents to consent to emergency medical treatment or services for the Participant if deemed necessary and agrees to be financially responsible for any medical expenses incurred.
VI. Photographic Release
The Participant and/or Parent/Guardian grants permission to the Organization to use photographs, video recordings, or likenesses of the Participant for promotional or educational purposes.
VII. Acknowledgment of Understanding
The Participant and/or Parent/Guardian acknowledges that they have carefully read this Waiver and Release of Liability, understand its contents, and voluntarily agree to its terms and conditions.
By signing below, the Participant and/or Parent/Guardian acknowledges and agrees to the terms and conditions of this Youth Sports Waiver And Release Of Liability.
