I. Declaration

I, [YOUR NAME], residing at [YOUR ADDRESS], being of sound mind and memory, and not acting under duress, menace, fraud, or undue influence, do hereby make, publish, and declare this to be my Last Will and Testament, revoking all prior wills and codicils.
