This Release Form ("Form") is entered into on
1. Purpose
The Participant agrees to participate in this activity, organized by the Company on June 15, 2050.
2. Assumption of Risk
The Participant acknowledges that the activity involves inherent risks, including but not limited to physical exertion, equipment hazards, and outdoor conditions. The Recipient voluntarily assumes all risks associated with the activity.
3. Release of Liability
In consideration for being permitted to participate in the activity, the Participant hereby releases, discharges, and holds harmless the Company from any and all claims, liabilities, damages, or demands arising out of or related to participation in the activity.
4. Indemnification

In consideration for signing this Form and participating in the activity, the Recipient acknowledges receipt of a gift voucher worth $50 redeemable at [Your Company Name].
