Pet Addendum For Rental Lease Agreement

This Pet Addendum ("Addendum") is made and entered into as of August 8, 2054, by and between [Your Name] ("Landlord") and George Michael ("Tenant"), and is hereby incorporated into and made part of the Rental Lease Agreement dated July 1, 2054, for the property located at 123 Maple Street, Springfield, IL 62701 ("Premises").

1. Pet Description and Approval

The Tenant has been granted permission to keep the following pet(s) on the Premises:

3. Pet Rules and Restrictions

The Tenant agrees to adhere to the following rules:

Pets must be kept on a leash or in a carrier when outside the unit.
Pets must be leashed or in a carrier in common areas.
Pets must not cause noise disturbances that can be heard from neighboring units.
Tenant must clean up after pets promptly and dispose of waste in designated areas.
Pets are not allowed to roam freely within the unit.

4. Liability and Insurance

The Tenant is fully responsible for any damage caused by the pet(s) to the Premises or common areas. The Tenant agrees to:

Get and keep renter's insurance with pet damage coverage.
Provide proof of insurance to the Landlord upon request.

5. Maintenance and Care

The Tenant agrees to:

Ensure proper pet care, including regular vet visits and vaccinations.
Maintain cleanliness of the unit and common areas related to pet care.
Notify the Landlord of any issues related to pet care or health immediately.

6. Termination and Removal Conditions

The Landlord reserves the right to request the removal of the pet(s) under the following conditions:

If the pet(s) cause damage to the Premises or create a nuisance.
If the Tenant fails to follow this Addendum's rules.
If the Tenant's insurance coverage lapses.

If the Landlord requires the removal of the pet(s), the Tenant must comply within 10 days of notification. Failure to do so may result in lease termination.
