HOUSING OMBUDSMAN COMPLAINT LETTER

August 12, 2051

Ombudsman Office
321 Birch Street
Maplewood, NY 12551

To whom it may concern,

I am writing to file a formal complaint against Maplewood Municipal Services regarding an issue with their waste management services. Despite several attempts to address this matter through their customer service department, I have not received a satisfactory resolution.

On June 10, 2051, I reported a missed garbage collection for my residence at 789 Oak Avenue. The scheduled collection date was June 9, 2051, and the missed service has led to significant inconvenience, including health and sanitation concerns due to the accumulation of waste.

I first contacted Maplewood Municipal Services on June 11, 2051, and was assured that a team would be dispatched to collect the missed waste within 24 hours. Despite this assurance, no collection occurred. I followed up on June 14, 2051, and spoke with a representative named Jarrett Bailey, who promised that the issue would be escalated and resolved by June 16, 2051. However, as of August 10, 2051, the missed collection has not been addressed, and my repeated follow-up calls have gone unanswered.

I seek a formal investigation and corrective action to prevent recurrence, along with a written apology and service assurance from Maplewood Municipal Services. Please review my complaint and facilitate a resolution. I trust your prompt and fair response. Thank you.

Yours sincerely,
