COMPLAINT LETTER TO DOCTOR

Dear Dr. Smith,
I am writing to express my concerns regarding the medical care I have received at HealthCare Clinic. I hope to address several issues I have experienced over the past few months, to improve my ongoing treatment.
Firstly, during my appointment on July 15, 2054, I felt that the consultation was inadequately thorough. Despite explaining my persistent symptoms of fatigue and dizziness in detail, I did not feel that my concerns were fully explored. The examination was brief, and there was little discussion about potential diagnostic tests or treatment options. This left me feeling uncertain about the next steps in managing my health.
Additionally, I encountered significant problems with the prescribed medication. On July 20, 2054, you prescribed Xylozine, which has resulted in several severe side effects, including persistent nausea, blurred vision, and headaches. These side effects were not discussed during our consultation, and I was not given any guidance on what to expect or how to manage these adverse effects. I had to contact the emergency room on August 1, 2054, due to the intensity of these symptoms.
I am unhappy with the lack of follow-up after my appointment on August 5, 2054. Despite assurances, I have not received any communication, leaving my treatment concerns unresolved. I highly value your expertise and would like to schedule a meeting to discuss and explore solutions. Thank you for your attention. I look forward to a prompt resolution.
Sincerely,
[Your Name]
