I. Introduction

This Power of Attorney is made on August 20, 2050, by John Smith (“the Principal”), residing at 123 Elm Street, Springfield, IL 62704. The Principal is executing this document to designate and authorize Jane Doe (“the Agent”), residing at 456 Oak Avenue, Springfield, IL 62705, to act on the Principal’s behalf in managing business operations during the Principal’s recovery from health issues.


II. Designation of Agent

The Principal hereby appoints Jane Doe as their true and lawful Agent with full power and authority to manage and conduct all business operations on behalf of the Principal. III. Effective Date and Duration

VI. Third-Party Reliance

All third parties dealing with the Agent may rely entirely on this Power of Attorney and are not required to inquire into the authority of the Agent or the validity of this document.


VII. Governing Law

This Power of Attorney shall be governed by and construed in accordance with the laws of the state of Illinois, and it is intended to be valid in all jurisdictions where it is presented.
