Introduction
This Release and Release of Liability Agreement (the “Agreement”) is made effective as of October 12, 2050, by and between [YOUR COMPANY NAME] ("Releasee") and Erika Lewis ("Releasor"), for the purpose of governing the rights and responsibilities related to the contractor services provided by the Releasor to the Releasee. Agreement

Release of Liability. This release extends to any physical or financial harm suffered by the Releasor as a result of the services provided to the Releasee.

Assumption of Risk. The Releasor acknowledges and understands the risks associated with the work being performed, including, but not limited to, risks of injury, property damage, and financial loss. The Releasor voluntarily assumes all risks associated with the performance of services under this Agreement.

Indemnification. The Releasor agrees to indemnify and hold harmless the Releasee from any claims, damages, or liabilities arising from any negligent acts or omissions by the Releasor in connection with the services provided under this Agreement.

No Employer-Employee Relationship. It is understood and agreed that the Releasor is providing services as an independent contractor and is not an employee of [YOUR COMPANY NAME]. The Releasor is solely responsible for any and all taxes, insurance, or other obligations incurred in connection with the provision of services.

Term and Termination. This Agreement shall remain in effect from the date of execution until December 31, 2050, or until terminated by either party upon written notice to the other. The release of liability provisions shall survive any termination of this Agreement.
