Florida Release of Liability

This Release of Liability (the “Release”) is made and entered into as of September 27, 2055, by and between [YOUR COMPANY NAME], located at [YOUR COMPANY ADDRESS] (the “Company”), and the undersigned participant (the “Participant”).

1. Acknowledgment of Risk

The Participant acknowledges that they are voluntarily participating in sports activities, including but not limited to tennis, basketball, and other recreational programs, which may involve physical exertion and inherent risks. The Participant understands that these risks may include, but are not limited to, personal injury, property damage, and/or death.

2. Release and Waiver

In consideration for being allowed to participate in the aforementioned activity/event, the Participant hereby releases, waives, discharges, and covenants not to sue [YOUR COMPANY NAME], its officers, agents, employees, and volunteers (collectively referred to as the "Released Parties") from any and all claims, demands, actions, or causes of action of any kind or nature, including but not limited to negligence, arising out of or related to the Participant's participation in the activity/event.

3. Indemnification

The Participant agrees to indemnify and hold harmless the Released Parties from any loss, liability, damage, or costs, including attorney’s fees, incurred as a result of any claims made against the Released Parties arising out of the Participant’s participation in the activity/event, whether caused by the negligence of the Released Parties or otherwise.

4. Medical Treatment

The Participant consents to receive medical treatment deemed necessary in the event of injury, accident, or illness during the activity/event. The Participant understands that they are responsible for any medical expenses incurred as a result of such treatment.

5. Governing Law

This Release shall be governed by and construed in accordance with the laws of the State of Florida. Any disputes arising out of or related to this Release shall be resolved in the courts located in Miami-Dade County, Florida.

6. Severability

If any provision of this Release is found to be unenforceable or invalid, the remaining provisions shall remain in full force and effect.

7. Acknowledgment of Understanding

The Participant has read this Release of Liability, fully understands its terms, and understands that they are giving up substantial rights, including the right to sue. The Participant acknowledges that they are signing the Release freely and voluntarily, and intend by their signature to be a complete and unconditional release of all liability to the greatest extent allowed by law.
