Liability Release

I, the undersigned owner of the above-mentioned dog, agree to the following terms and conditions:

1. Health and Safety: I certify that my dog is in good health and up to date on vaccinations. I acknowledge that grooming can sometimes expose hidden medical conditions or exacerbate existing ones.

2. Aggressive Behavior: I understand that if my dog behaves aggressively or bites during the grooming session, the groomer may discontinue services for the safety of my pet and the staff.

3. Matting and Shaving: I acknowledge that if my dog’s coat is matted, the groomer may need to shave the affected areas, which may cause skin irritation. I accept that this may be necessary for the comfort and health of my dog, and I will not hold the groomer liable for any effects of shaving.

4. Injury and Emergencies: While every effort is made to ensure the safety of all pets, accidents can happen. I understand that in the unlikely event of injury or emergency, I will be contacted immediately.

5. Payment and Cancellation: I agree to pay in full for the grooming services at the time of pick-up. I understand that cancellations must be made 24 hours in advance, or I may be charged a cancellation fee.

By signing below, I acknowledge that I have read and understood the terms and conditions of this release form. I release the groomer and the grooming salon from any and all liability, damages, or claims related to the grooming of my dog.
