Employee Training Waiver
EMPLOYEE TRAINING WAIVER
I. Introduction
This Employee Training Waiver (hereinafter referred to as the "Waiver") is entered into by and between [YOUR COMPANY NAME] (hereinafter referred to as the "Company") and the undersigned employee (hereinafter referred to as the "Employee") for the purpose of participating in safety training involving physical activities, machinery operation, and/or hazardous materials.
II. Acknowledgment of Risks
By signing this Waiver, the Employee acknowledges the following:

Participation in safety training may involve inherent risks, including but not limited to:

Physical injury (e.g., cuts, bruises, sprains)

Property damage (e.g., equipment damage)

Exposure to hazardous materials (e.g., chemicals, noise)

Equipment malfunction or failure

The Employee understands these risks and voluntarily accepts responsibility for any injuries or damages incurred during training.
III. Release of Liability
The Employee hereby releases, waives, and discharges the Company and its officers, employees, agents, and assigns from any and all claims, demands, or causes of action arising from or related to the Employee's participation in the safety training, including claims based on negligence.
IV. Employee Responsibilities
The Employee agrees to:

Follow all safety protocols and instructions provided during the training.

Use personal protective equipment (PPE) as required (e.g., hard hats, safety goggles, gloves).

Immediately report any unsafe conditions or incidents to the training instructor or supervisor.

Participate actively in the training and engage in all activities as instructed.
V. Indemnification
The Employee agrees to indemnify and hold harmless the Company from any claims, losses, damages, or expenses (including legal fees) arising out of or related to the Employee’s participation in the safety training.
VI. Medical Clearance
The Employee confirms that they:

Are physically fit to participate in the training.

Have no medical conditions that would prevent safe participation.

Agree to provide any necessary medical clearance documentation if requested by the Company.
VII. Governing Law
This Waiver shall be governed by and construed in accordance with the laws of the State of California.
VIII. Acknowledgment and Agreement
By signing this Waiver, the Employee acknowledges that they have read and understood its contents, and voluntarily agree to the terms and conditions set forth herein.
