1. Acknowledgment of Risks
I, the undersigned employee, acknowledge that participation in company-sponsored events, including but not limited to team-building activities, retreats, and sports events, involves inherent risks. These risks may include, but are not limited to:

Accidents or incidents (e.g., vehicle accidents)
2. Waiver and Release of Liability
By signing this waiver, I agree to release, indemnify, and hold harmless [YOUR COMPANY NAME], its employees, agents, officers, and representatives from any and all claims, liabilities, damages, or expenses, including legal fees, arising out of or in connection with my participation in the above-mentioned event, even if caused by the negligence of the Company.
3. Assumption of Responsibility
