Acknowledgment of Risks

I, the undersigned, hereby acknowledge that my child, Gino Rohan, wishes to participate in activities at Happy Trails Play Center. I understand that there are inherent risks associated with participation in play activities, including but not limited to:

  • Slips, trips, and falls

  • Collision with other participants

  • Injuries resulting from the use of playground equipment, such as slides, climbing structures, and inflatable bounce houses

Waiver and Release of Liability

In consideration of my child being allowed to participate in these activities, I hereby agree to the following:

  1. Waiver and Release of Liability:
    I, on behalf of myself and my child, do hereby release, waive, and discharge Happy Trails Play Center, its owners, employees, and agents from any liability for any injuries or damages that may occur during participation in the Indoor Playground activities.

  2. Assumption of Risk:
    I acknowledge that I have been informed of the potential risks involved, including the possibility of minor injuries like scrapes and bruises or more serious injuries, and voluntarily assume all risks of injury or damage associated with my child's participation.

  3. Medical Release:
    In the event of an emergency, I authorize Happy Trails Play Center to seek medical assistance for my child, and I agree to be responsible for any medical expenses incurred, including transportation to a medical facility if necessary.

  4. Photography and Video Release:
    I grant Happy Trails Play Center permission to take photographs and videos of my child during activities and to use them for promotional purposes, including on social media platforms, newsletters, and website content.

  5. Conduct Policy:
    I understand that my child is expected to follow all safety rules and guidelines provided by Happy Trails Play Center staff, and that failure to do so may result in my child's removal from the premises without a refund.
