Printable Personal Training Agreement
This Personal Training Agreement ("Agreement") is entered into on this [Date], by and between:
1. Parties
Trainer: [Your Name], residing at Pittsburgh, PA 15201.
Client: Beverly Haley, residing at St. Paul, MN 55101.
3. Purpose
The purpose of this Agreement is to establish the terms and conditions under which the Trainer will provide personal training services to the Client.
4. Terms and Conditions
1. Services Provided
The Trainer agrees to provide personal training services, which include but are not limited to, personalized exercise programs, fitness assessments, nutritional guidance, and on-going support.
2. Duration of Agreement
This Agreement shall commence on [Start Date] and will continue until [End Date], unless terminated earlier as per the terms outlined in this Agreement.
3. Fees and Payment
The Client agrees to pay the Trainer a fee of [Amount] per session. Payment is due on [Payment Due Date], and any missed sessions will be charged unless a 24-hour cancellation notice is provided by the Client. The Trainer reserves the right to adjust the fees with prior notice.
4. Client Responsibilities

5. Liability Waiver
The Client understands and accepts the risks involved in physical activities and agrees to release the Trainer from any liability in connection with personal training sessions. The Client acknowledges that they have been advised to seek medical clearance prior to starting the training program.
6. Termination
Either party may terminate this Agreement by providing a written notice of [Notice Period] days. In the event of a breach of terms by either party, this Agreement may be terminated immediately by the other party.
7. Confidentiality
Both parties agree to maintain confidentiality of all information exchanged during the course of this Agreement, except where disclosure is required by law.
