NDIS Service Agreement
1. Introduction
This NDIS Service Agreement is made between the Service Provider and the Participant in accordance with the National Disability Insurance Scheme Act 2050. The Agreement sets out the terms and conditions under which the Service Provider will deliver supports to the Participant.

3. Purpose of the Agreement
The purpose of this Agreement is to specify the desired outcomes for the Participant and how the Service Provider will deliver supports to assist the Participant in achieving these goals. It ensures clarity and understanding between the Participant and the Service Provider in terms of responsibilities and expectations.
4. Support Schedule
The Service Provider agrees to provide the supports as outlined in the attached schedule (Schedule A), which details the types of services, duration, and frequency of support.
5. Responsibilities
5.1 Service Provider Responsibilities
The Service Provider agrees to:

Obtain the Participant's consent before sharing personal details.
5.2 Participant Responsibilities
The Participant agrees to:

Respect the rights of service providers.

Notify the Service Provider of any changes in circumstances.

Pay for the agreed services according to the terms.
6. Payment Terms
The Service Provider will seek payment for their services directly from the National Disability Insurance Scheme provider, as agreed upon in this Service Agreement and supported by the Participant's plan.
7. Cancellation Policy
Either party must provide [number] days' notice for the cancellation of scheduled services. Failure to provide adequate notice may result in cancellation fees as outlined in the Schedule of Supports.
8. Dispute Resolution
In case of any disagreements, parties agree to communicate openly and collaboratively to resolve issues. If unresolved, the matter will be escalated to the relevant governing body as per the NDIS guidelines.
9. Signatures
By signing this Agreement, both parties acknowledge they understand and agree to the terms and conditions outlined above.
