Acknowledgment of Risks
I understand and acknowledge the following:
Tanning may result in skin sensitivity, dryness, or irritation.
Prolonged or frequent exposure to UV light may increase the risk of skin damage.
Certain medications and skincare products may increase sensitivity to UV light. I confirm that I have informed the staff of any medications or products I am using.
Consent and Waiver
I hereby agree to the following:
I understand the risks associated with tanning and assume full responsibility for any outcomes resulting from the service.
I release the company, its employees, and agents from any liability related to injuries or adverse effects caused by tanning services.
I will follow all instructions and guidelines provided by the staff to ensure my safety during and after the tanning session.
