1. Services Provided
The Provider agrees to provide child care services for the Child listed above under the terms outlined in this Agreement. Care will include:

4. Health and Safety

Illness Policy: Children displaying symptoms of illness (e.g., fever, vomiting, diarrhea, rash) will not be admitted to care to ensure the health and safety of others. The Child may return after being symptom-free for at least 24 hours or upon providing a doctor’s note confirming they are not contagious.

Medication Administration: The Provider will only administer medication with prior written consent from the Parent/Guardian. Medications must be in their original packaging with clear dosage instructions.

Emergency Contact: The Parent/Guardian must provide a current list of emergency contacts and notify the Provider of any changes immediately. Emergency contact information will be used only in cases of urgency.

Accidents and Incidents: In the event of an accident or incident, the Provider will notify the Parent/Guardian immediately and provide a written report detailing the occurrence and any actions taken.
5. Meals and Nutrition

7. Termination and Withdrawal

The Provider will maintain regular communication with the Parent/Guardian, including:

10. Governing Law
