1. I am over the age of eighteen (18) years and competent to make this affidavit regarding the joint tenancy of the property described below.

I understand that this affidavit is being provided to document my sole ownership of the Property and may be relied upon by third parties, including, but not limited to, title companies, mortgage lenders, real estate professionals, and others involved in transactions concerning the Property. I acknowledge that any false statement in this affidavit may result in legal penalties, including, but not limited to, civil liability, criminal prosecution, and invalidation of any property transaction or claim.
   

8. This affidavit is executed for the sole purpose of confirming that I, as the surviving joint tenant, have succeeded to the ownership of the Property and that the interest of the Deceased Tenant in the Property is no longer valid, thereby fully vesting ownership in me, free of any claims of the Deceased Tenant’s heirs or successors.

I hereby certify under penalty of perjury under the laws of the State of Illinois that the foregoing statements are true and correct.
