1. Personal Competency
   I am over the age of 18 years, of sound mind, and fully competent to make this affidavit based on personal knowledge of the facts set forth herein.
   

2. Joint Tenancy Relationship
   I was a joint tenant with the right of survivorship with John Doe concerning the property located at 456 Oak Avenue, Springfield, IL 62701, as evidenced by the deed recorded on March 15, 2040, in the Recorder of Deeds Office, Sangamon County, Illinois. A certified copy of the recorded deed is attached hereto as Exhibit A.
   

3. Death of Joint Tenant
   John Doe passed away on July 12, 2050, as evidenced by the attached certified copy of the death certificate, which is marked as Exhibit B.
   

4. Surviving Tenant Ownership
   Upon the death of John Doe, by operation of the law governing joint tenancy with rights of survivorship, I became the sole owner of the property located at 456 Oak Avenue, Springfield, IL 62701.
   

5. Estate Administration and Other Claims
   To the best of my knowledge and belief:
   

   • No proceedings have been instituted for the administration of the estate of John Doe in any court.
     

   • No other person has a legal interest, claim, or encumbrance in or to the property described above.
     

6. Truthfulness of Statements
   I affirm under penalty of perjury that the foregoing statements are true and correct to the best of my knowledge, information, and belief.
