MEDICAL AUTHORIZATION

In the event of an emergency, I authorize the Releasee or their designated agents to seek medical treatment on my behalf if I am unable to make decisions regarding my own care. I agree to bear any and all costs associated with such treatment.

GOVERNING LAW

This form shall be governed by and construed in accordance with the laws of the United States and the state of [State].

ENTIRE AGREEMENT
