Sports Release of Liability Form
Please read this form carefully. By signing, you are agreeing to waive certain legal rights.
RELEASE OF LIABILITY
I, the undersigned [Releasor Name]("Releasor"), hereby release, waive, discharge, and hold harmless the [Releasee Name]("Releasee"), its agents, employees, officers, directors, volunteers, and affiliates from any and all claims, demands, actions, causes of action, damages, or liability of any kind arising from or in connection with my participation in the above-described sporting activity or event.
ASSUMPTION OF RISKS
I acknowledge and understand that participating in sports involves certain inherent risks, dangers, and hazards that could result in injury, death, or property damage. These risks may include, but are not limited to:
I voluntarily and knowingly assume all such risks associated with my participation in the activity, whether or not the risks are listed above.
MEDICAL AUTHORIZATION
In the event of an emergency, I hereby authorize the Releasee or its designated agents to seek medical treatment on my behalf should I be unable to make decisions regarding my own care. I understand that I will be responsible for any medical costs incurred.
NO ADMISSION OF LIABILITY
This release does not constitute an admission of liability or fault by the Releasee. It simply serves to limit their liability as agreed upon by both parties.
ENTIRE AGREEMENT
This document represents the complete and entire agreement between the parties regarding the release of liability for the sporting activity and supersedes any prior agreements or understandings.
