Trampoline Waiver

I. Introduction

- Risk of collision with other participants

- Risk of falling from the trampoline surface
The Participant agrees that [YOUR TRAMPOLINE PARK NAME] shall not be held liable for any injuries or damages incurred during the use of trampoline facilities, except in cases of gross negligence or willful misconduct on the part of the Park.

IV. Participant's Responsibilities

The Participant agrees to:

Refrain from engaging in reckless or dangerous behavior while using the trampoline facilities.
Use trampoline facilities only while in good physical condition and free from any health conditions that may increase the risk of injury.

V. Indemnification

The Participant agrees to indemnify and hold harmless [YOUR TRAMPOLINE PARK NAME] from any loss, liability, damage, or cost, including attorney fees, arising out of or related to the Participant's use of the trampoline facilities or the violation of any law or the rights of a third party.

VI. Participant's Health and Safety

Participant affirms that they are in good health and do not suffer from any medical condition or disability which may affect their ability to safely engage in trampoline activities. It is the Participant's responsibility to seek medical advice if they have any health concerns prior to participating.

VII. Waiver and Release of Liability

VIII. Agreement to Follow Directions

If the Participant is under the age of 18, this document must be reviewed and signed by a parent or legal guardian. The parent or guardian agrees to be bound by these terms and consents to the minor's participation in the trampoline activities.

X. Photo and Video Release
