I. Property Details

This Beneficiary Deed ("Deed") is made this 6th day of October 2060 by and between [Your Name] ("Grantor"), residing at Arvada, CO 80005, and Gary Elliott ("Beneficiary"), residing at Arvada, CO 80005.

II. Recitals

WHEREAS, Grantor is the owner of the following described real property situated in the City of Arvada, County of Jefferson, State of Colorado ("Property"): "Lot 5, Block 3, Maplewood Subdivision, according to the plat thereof recorded in Plat Book 15, Page 67, in the office of the County Recorder of Jefferson County, Colorado.", and

WHEREAS, Grantor desires to transfer this Property to Beneficiary upon Grantor's death.

III. Conveyance

NOW, THEREFORE, Grantor hereby conveys and quitclaims to Beneficiary, effective on the death of Grantor, all right, title, and interest in and to the Property, subject to the following conditions:

1. This Deed shall not be effective unless recorded prior to Grantor's death in the office of the County Recorder of Jefferson County.

2. Grantor retains the ownership of the Property during Grantor's lifetime and may revoke this Deed at any time before Grantor's death by recording an instrument of revocation in the office of the County Recorder of Jefferson County.

3. This conveyance is subject to all easements, right of ways, restrictions, and covenants of record.
