1. Introduction

This Support Worker Service Agreement ("Agreement") is entered into between [Your Company Name] ("Provider") and [Support Worker Name] ("Support Worker") on July 15, 2050.

2. Scope of Services

2.1 The individual working in the role of Support Worker has agreed to provide support services to clients, their responsibility is to do so following the assignments given by the Provider.

2.2 The services that are expected to be provided might encompass various extents and should not be perceived as being strictly limited to the ones mentioned. Instead, we can accommodate a broader spectrum that might not necessarily be part of the list.

  • Assisting clients with activities of daily living (ADLs) such as bathing, grooming, and dressing.

  • Assisting with meal preparation and feeding.

  • Administering medication as per the client's care plan.

  • Accompanying clients to medical appointments or recreational activities.

  • Providing companionship and emotional support to clients.

  • Performing light housekeeping tasks as necessary.

3. Duties and Responsibilities

3.1The responsibilities of a Support Worker are as follows:

  • Provide services professionally and compassionately.

  • Respect and maintain the confidentiality of clients' information.

  • Adhere to all policies and procedures of the Provider.

  • Maintain accurate records of services provided and any observations made regarding the client's condition.

  • Report any changes in the client's condition or concerns to the appropriate supervisor.

3.2 Provider Responsibilities:

  • Assign clients and provide necessary training and orientation to the Support Worker.

  • Provide necessary support and resources to enable the Support Workers to perform their duties effectively.

  • Address any concerns or issues raised by the Support Worker on time.

  • Ensure compliance with all relevant regulations and standards.

4. Term and Termination

4.1 This Agreement shall officially start or take effect on the specified Effective Date mentioned herein, and it shall continue and remain in force until such time that it is terminated or ended by one of the parties involved, either the first party or the second party.

4.2 This Agreement may be terminated by either party, given that they provide written notice to the opposing party a minimum of [notice period] days in advance of the intended termination date.

4.3 The Provider retains the right, or holds on to the power, to effectively end, or bring to a termination, the existing agreement we have at once, without any delay, under circumstances such as the Support Worker violating, or not abiding by, any of the stipulated terms or conditions that make a part of this agreement or under any other fair and justified reason that is deemed acceptable and valid by the Provider.

5. Compensation

5.1 Keeping in mind, acknowledging, and taking into account the services that are being provided by the Support Worker, it is decided and stated that the Provider, the one who is receiving these services, shall compensate and remunerate the Support Worker.

The agreed rate at which this payment will be made by the Provider to the Support Worker has been established as [rate] for each hour of service provided by the Support Worker.

5.2 The frequency of the payment shall be as stated unless there is a written agreement that states a change in the frequency of the payment.

6. Confidentiality

6.1 The Support Worker gives their agreement to uphold the confidentiality of all the information regarding the client. Furthermore, they also comply with every privacy law and regulation that is relevant to ensure the utmost privacy and security of the client's information.

6.2 The Support Worker shall not disclose any confidential information obtained while providing services under this Agreement without the client's express consent or as required by law.

7. Governing Law

7.1 This Agreement is to be governed by and interpreted or understood under the prevailing laws of the specified jurisdiction, [Jurisdiction].

8. Entire Agreement

8.1 This Agreement stands as the whole and complete understanding between all involved parties concerning the subject matter being discussed within this Agreement.

It overrules and supersedes all previous agreements, representations, and understandings. This includes not just written agreements, but oral agreements as well; all prior understandings or claims are to be considered null and void, with this Agreement now serving as the sole authoritative document between the parties.

9. Amendment

9.1 This Agreement can only be altered or modified through a process that demands the changes to be documented in written form and must also be duly signed by the two parties involved.

As a sign of their acceptance and adherence, the aforementioned parties have endorsed and carried out this agreement, in effect from the date noted in the opening of this document.
