This Telecommuting Agreement Form ("Agreement") is made and entered into on
The Employee agrees to comply with all company policies and expectations regarding performance, communication, and confidentiality while telecommuting. The Employee acknowledges that this Agreement does not change the at-will employment status.
The Employee agrees to work remotely according to the following schedule:
The Employee understands that any modifications to this schedule must be approved in advance by the Employer.
The Employer will provide the following equipment/resources to support telecommuting:
The Employee agrees to take reasonable care of all equipment provided and to return it promptly upon termination of the Agreement.
This Agreement shall remain in effect unless terminated earlier by either Party. The Employer reserves the right to terminate or modify the telecommuting arrangement at its discretion with reasonable notice to the Employee.
By signing below, both Parties agree to the terms and conditions outlined in this Agreement.
