Please review and complete this agreement to establish telecommuting terms.

I. The Parties

This Telecommuting Agreement Form ("Agreement") is made and entered into on Month Day, Year by and between [Your Company Name], with a primary place of business at [Your Company Address] ("Employer"), and Employee's Name, with a primary address at Employee's Address ("Employee"). The Employer and Employee may collectively be referred to as the "Parties" or individually as a "Party."

II. Terms and Conditions

The Employee agrees to comply with all company policies and expectations regarding performance, communication, and confidentiality while telecommuting. The Employee acknowledges that this Agreement does not change the at-will employment status.

III. Telecommuting Schedule

The Employee agrees to work remotely according to the following schedule:

      IV. Equipment and Resources

      The Employer will provide the following equipment/resources to support telecommuting:

        The Employee agrees to take reasonable care of all equipment provided and to return it promptly upon termination of the Agreement.

        V. Term and Termination

        This Agreement shall remain in effect unless terminated earlier by either Party. The Employer reserves the right to terminate or modify the telecommuting arrangement at its discretion with reasonable notice to the Employee.
