Letter of Accident Report

Dear Ms. Artiaga,
I am writing to provide a detailed account of the accident that occurred on November 15, 2023, at approximately 3:30 PM at the intersection of Bassel Street and Oak Avenue in Metairie.
On the mentioned date, I was driving my vehicle, a 2018 Honda Accord with license plate KAJ123, proceeding through the intersection when a black SUV, identified as a Chevrolet Tahoe with license plate OFL789, failed to yield the right of way and collided with the front passenger side of my vehicle. The weather conditions at the time were clear with no precipitation.
The collision resulted in significant damage to the front passenger side of my vehicle and minor injuries to my passenger, Matthilda Cooke. She sustained minor cuts and bruises and received medical attention at Metairie Medical Center. I have attached photographs depicting the scene, damages to both vehicles, and a copy of the medical report for Matthilda Cooke.
I promptly contacted the local authorities, and a police report (Report Number: 25641) was filed at the scene by Officer Smith. Matthilda Cooke and I obtained statements from two witnesses, Emily White and Robert Davis, both of whom have agreed to cooperate with the investigation.
I have also attached a copy of my driver's license, vehicle registration, and insurance information for your records.
