I am writing to you with concern about your recent performance at Life Link. It has come to my attention that your work performance has not been at the level expected of someone in your role. We have noticed a significant decline in the output and quality of your work over the past couple of months. This is concerning considering the vital role you play in our company's operations.
We value your contributions to our team and believe that you can overcome this setback. We'd like to extend our support in finding ways to improve and get back on track. In the forthcoming weeks, I propose having regular one-on-one meetings to discuss achievable goals and construct a specific plan for performance improvement.
