This Childcare Agreement ("Agreement") is made and entered into on September 13, 2060, by and between:

2. Purpose
This Agreement sets forth the terms and conditions under which the Childcare Provider agrees to provide childcare services to the children listed below, for the Parent/Guardian(s) identified above.
3. Children Covered by Agreement
This Agreement applies to the following child/children:

4. Duration of Services
This Agreement shall commence on September 13, 2060, and will continue until terminated as outlined in Section 10. The services will be provided on the following days and times:

Monday: 7:30 AM to 6:00 PM

Tuesday: 7:30 AM to 6:00 PM

Wednesday: 7:30 AM to 6:00 PM

Thursday: 7:30 AM to 6:00 PM

Friday: 7:30 AM to 6:00 PM
-
Weekends: No Services Provided
5. Fees and Payment Terms
The Parent/Guardian(s) agree to pay the Childcare Provider the following fees:

Weekly Fee: $300 per child
-
Overtime Rate: $25 per hour for care provided beyond regular hours.
Payment is due weekly on Friday by 5:00 PM. Late payments will incur a late fee of $10 for each day payment is delayed.
6. Services Provided
The Childcare Provider agrees to provide the following services:

Supervision and care of children during the agreed-upon hours.

Nutritional meals and snacks.
-
Assistance with homework as needed.
-
Age-appropriate activities, both educational and recreational.
-
Nap/rest time as appropriate for the child’s age.
-
Administration of prescribed medication (with written consent from Parent/Guardian).
7. Parent/Guardian Responsibilities
The Parent/Guardian(s) agree to:

Provide necessary items such as diapers, extra clothing, and any special dietary requirements.

Pick up the child/children on time, according to the agreed-upon schedule.

Inform the Childcare Provider of any changes in the child’s health, behavior, or routine.

Provide up-to-date emergency contact information.
-
Notify the Childcare Provider of any absences or late arrivals as early as possible.
8. Health and Safety
The Childcare Provider will take reasonable steps to ensure the health and safety of all children in their care, including but not limited to:

Maintaining a clean and safe environment.
-
Supervising outdoor play and ensuring all equipment is age-appropriate.

Administering first aid when necessary and promptly notifying the Parent/Guardian(s) of any injuries.

Requiring immunization records and emergency contact information.
-
Following any health-related restrictions or special instructions provided by the Parent/Guardian(s).
9. Termination of Agreement
This Agreement may be terminated by either party with two weeks' written notice. Immediate termination may occur if:

The Parent/Guardian(s) fail to make payments as outlined.

The Childcare Provider determines that continued care poses a risk to the health, safety, or well-being of the child or other children in care.

Either party breaches a material term of this Agreement.
10. Absences and Vacation

If the child is absent due to illness, the Parent/Guardian(s) agree to notify the Childcare Provider as soon as possible.

Vacation time for the Childcare Provider: Two weeks of paid vacation annually, during which no services will be provided. The Parent/Guardian(s) will receive notice at least four weeks in advance.

Vacation time for the child: The Parent/Guardian(s) must provide at least two weeks' notice of any planned absences. Full payment is still required for up to two weeks of vacation taken by the child.
11. Emergency Procedures
In case of emergency, the Childcare Provider will attempt to contact the Parent/Guardian(s) immediately. If the Parent/Guardian(s) cannot be reached, the Childcare Provider is authorized to contact the following emergency contact:

The Childcare Provider is also authorized to obtain emergency medical treatment for the child in the event that the Parent/Guardian(s) cannot be reached in a timely manner.
12. Confidentiality
The Childcare Provider agrees to maintain the confidentiality of all personal and sensitive information regarding the Parent/Guardian(s) and their child/children. This includes information related to the child’s health, behavior, and any other personal details.
13. Governing Law
This Agreement shall be governed by and construed in accordance with the laws of the state of Illinois, without regard to its conflict of law principles.
14. Dispute Resolution
Any disputes arising from this Agreement shall be resolved through mediation before any legal action is taken.
15. Amendments
This Agreement may be amended only by a written agreement signed by both the Parent/Guardian(s) and the Childcare Provider.
16. Entire Agreement
This Agreement constitutes the entire understanding between the parties with respect to childcare services and supersedes all prior discussions or agreements.
